The passage appears at the beginning of the story, so the narrator does seem to be sane at this point. Much of the passage indicates the thought processes of the narrator, but he seems relatively rational. By the end of the passage, though, the reader can see that the narrator is beginning his descent into madness with the statement, "By long suffering my nerves had been unstrung, until I trembled at the sound of my own voice." This sort of behavior is not typically seen in mentally healthy individuals.
